CH Care OnDemand enables ACE (non-Union) associates covered by the First Choice HealthCare Plan to see a clinician anytime, from anywhere, by phone or secure online video – all through the CH Care OnDemand app.

• Service includes diagnosis, treatment and prescriptions, depending on condition
• Consultations intended for non-emergency issues such as:
o Cold and Flu Symptoms
o Seasonal Allergies
o Constipation
o Cough
o Sore Throat
o Diarrhea/Vomiting
o Dizziness
o Earache
o Headache
o Skin Conditions
o Heartburn
o Sprains and Strains
o Nosebleed
o Red Eye
o Sinus Problems
o Urinary Problems

How does it work?

Open the CH Care on Demand app, request a visit and provide your medical history. Within minutes, a board certified clinician will review your medical history and contact you via phone or video for a consultation/visit. The doctor will recommend next steps and/or submit a prescription to your local pharmacy.

NOTE:
• Dependent Children under the age of 2 are not eligible to receive care through CH Care OnDemand.
• Dependent Children on Catholic Health’s First Choice HealthCare Plan must be between the ages of 18 – 26 to request care through CH Care OnDemand without parent(s)/legal guardian(s) present.
• Canadian Associates who are enrolled in the First Choice HealthCare Plan are not permitted to use Telehealth services in Canada. At this time CH Care OnDemand isn’t offered internationally.

If you are experiencing more serious symptoms, please go to your nearest ER for immediate treatment. These symptoms may include chest pain; uncontrolled bleeding; severe or sudden pain; coughing or vomiting blood; sudden dizziness, weakness, change in vision, slurred speech; severe or persistent vomiting or diarrhea; or changes in mental status, such as confusion.
